This information is collected via our OVATU software; over the phone, in salon, email or our social media channels. Consultations & consent forms are an essential part to being pampered within Skye Serenity to ensure no treatments are carried out to cause a negative effect on your health along with complying with legal & statutory legislations. Consent through a signature will always be required for us to obtain & process your health information. We will always ask for consent prior to obtaining any personal information. You can choose not to provide your personal details or health information however; we would be unable to provide products & treatments.

WHY DO SKYE SERENITY SPA NEED TO OBTAIN THIS INFORMATION?
In order to receive treatments & create a profile at Skye Serenity some personal information is required from you. Here at Skye Serenity your privacy is very important to us & only use the information collected to provide suitable treatments. Skye Serenity does not share your personal information with any third party without your prior consent, other than to process card transactions with your authority using iZettel LTD. Sensitive card information is not recorded or stored by Skye Serenity. All third parties are stated in this privacy notice or as part of our legal obligations under the relevant data protection laws.
As stated above questions maybe required relating to your health & medical history, examples: allergies, pregnancy, medication or injury etc that may contra-indicate you having treatments.
At any time you may request a copy of the personal information recorded about you, this can be modified by you at any time & you may also request all identifiable information with respect to yourself is withdrawn. Please be aware this may affect the treatments we are able to provide you.
All details are stored in Skye Serenity's booking system OVATU which has bank grade security.
Skye Serenity conforms to the new GDPR (General Data Protection Regulation) law.
Skye Serenity does not undertake phone, mail, email or SMS marketing without you first providing consent. For example Skye Serenity may send marketing campaigns relating to your birthday, if you haven’t visited Skye Serenity for some time, special event days or special offers. After visiting Skye Serenity an email or SMS asking you to rate our services & provide feedback maybe sent. This consent form is within our consultation form upon your first visit to Skye Serenity Spa.
TERMS & CONDITIONS
At Skye Serenity we are required to obtain certain personal & health information to enable us to perform treatments within our Spa. All information you provide us with must be true & correct, therefore any treatments carried out are taken at your own risk. You are aware that it is your responsibility to inform your therapist of any health & medical conditions.
By agreeing to the information you have given us you have an understanding that failure to disclose information requested in our consultation & consent forms may result in adverse side effects or reactions, therefore you accept full liability.

COVID 19 POLICY
All Skye Serenity Spa clients will be provided with our Covid-19 Policy via email once a treatment has been booked. To allow a clear understanding of the procedures we are putting in place not only to protect ourselves but importantly our clients.
